We sought to determine whether fall risk in MCI is associated with decline in higher\u2010level brain gait control.<\/p>\n<\/section>\n<section id=\"jgs16265-sec-0002\" class=\"article-section__content\">\n<h3 id=\"jgs16265-sec-0002-title\" class=\"article-section__sub-title section1\">DESIGN<\/h3>\n<p>Longitudinal study.<\/p>\n<\/section>\n<section id=\"jgs16265-sec-0003\" class=\"article-section__content\">\n<h3 id=\"jgs16265-sec-0003-title\" class=\"article-section__sub-title section1\">SETTING<\/h3>\n<p>Community\u2010dwelling adults from the Gait and Brain Study Cohort.<\/p>\n<\/section>\n<section id=\"jgs16265-sec-0004\" class=\"article-section__content\">\n<h3 id=\"jgs16265-sec-0004-title\" class=\"article-section__sub-title section1\">PARTICIPANTS<\/h3>\n<p>A total of 110 participants, aged 65\u2009years or older, with MCI.<\/p>\n<\/section>\n<section id=\"jgs16265-sec-0005\" class=\"article-section__content\">\n<h3 id=\"jgs16265-sec-0005-title\" class=\"article-section__sub-title section1\">MEASUREMENTS<\/h3>\n<p>Biannual assessments for medical characteristics, cognitive performance, fall incidence, and gait performance for up to 7\u2009years. Seven spatiotemporal gait parameters, including variabilities, were recorded using a 6\u2010meter electronic walkway. Principal components analysis was used to identify independent gait domains related to higher\u2010level (pace and variability domains) and lower\u2010level (rhythm domain) brain control. Associations between gait decline and incident falls were studied with Cox regression models adjusted for baseline covariates.<\/p>\n<\/section>\n<section id=\"jgs16265-sec-0006\" class=\"article-section__content\">\n<h3 id=\"jgs16265-sec-0006-title\" class=\"article-section__sub-title section1\">RESULTS<\/h3>\n<p>Of participants enrolled, 40% experienced at least one fall (28% of them with injuries) over a mean follow\u2010up of 31.6\u2009\u00b1\u200923.9 months. From the pace domain, slower gait speed (adjusted hazard ratio [aHR] per 10\u2010cm\/s decrease = 4.62; 95% confidence interval [CI] =\u20091.84\u201011.61;\u00a0<i>P\u00a0<\/i>= .001) was associated with severe injurious falls requiring emergency room (ER) visit; from the variability domain, stride time variability (aHR per 10% increase during follow\u2010up = 2.17; 95% CI = 1.02\u20104.63;\u00a0<i>P\u00a0<\/i>= .04) was associated with higher risk of all injurious falls. Rhythm domain was not associated with fall risk. Decline in pace domain was significantly associated with falls with ER visit (aHR = 3.67; 95% CI = 1.46\u20109.19;\u00a0<i>P\u00a0<\/i>= .005). After adjustments for multiple comparisons, gait speed and pace domain remained significantly associated with falls with ER visits. No statistically significant associations were found between gait domains and overall falls (<i>P\u00a0<\/i>\u2009\u2265\u2009.06).<\/p>\n<\/section>\n<section id=\"jgs16265-sec-0007\" class=\"article-section__content\">\n<h3 id=\"jgs16265-sec-0007-title\" class=\"article-section__sub-title section1\">CONCLUSION<\/h3>\n<p>Higher risk of injurious falls in older adults with MCI is associated with decline in gait parameters related to higher\u2010level brain control.<\/p>\n<\/section>\n<\/div>\n","protected":false},"author":19,"featured_media":0,"template":"","meta":{"_acf_changed":false},"studies-relation":[],"class_list":["post-4575","ccna_publication","type-ccna_publication","status-publish","hentry"],"acf":[],"yoast_head":"<!-- This site is optimized with the Yoast SEO plugin v28.2 -
